Forms
The Judicial Council approved forms at the urging of its Family and Juvenile Law Advisory Committee, which concluded that limited scope representation helps self-represented litigants. MMD will assist you with:
- accurate preparation of documents;
- preparation of your case so that you are placed in a better position than if left on your own;
- Obtain representation for portions of their cases, such as court hearings; and
- Obtain assistance in preparing, understanding and enforcing court orders.
The list below contains forms for your use in self representation. We can also provide various services in support of your case through limited scope representation or full representation. The forms are separated by categorized headings to assist in searching and ease of use. Simply click on each heading to reveal the forms associated with that category. Once the appropriate form has been located, you will be able to click the form name to view and download. All forms on the MMD website have been made to be filled in on your computer. You may also print and handwrite the form as well.
